Prospective Staff Questionnaire/Application

We ask that all who have understood employment with FCA and read the overview of working with FCA complete our questionnaire/application if you’re interested in a ministry job with FCA. Please specify the current position you are interested in and your questionnaire will be reviewed and forwarded to the local staff in the area. Please note incomplete questionnaires will not be forwarded.

The questionnaire will take more than one hour to complete. Unfortunatley there is not a save feature and the form needs to be completed in one setting.
